Clinical Nurse Case Coordinator

Key Responsibilities:

  • Deliver exceptional patient care aligned with the mission and values of Virginia B. Andes Volunteer Community Clinic (VBA).
  • Uphold the highest standards of healthcare practice, ethics, and professionalism.
  • Assess, plan, implement, monitor, and evaluate the options and services required to meet an individual’s health needs.
  • Coordinate care across the healthcare continuum to optimize clinical and financial outcomes.
  • Review and evaluate laboratory results, imaging studies, and hospital records based on orders from providers, to support timely and appropriate care planning and need for referrals.
  • Identify abnormal findings and communicate them to the appropriate care team members to ensure prompt intervention.
  • Serve as a liaison between patients, volunteers, physicians, and the interdisciplinary care team.
  • Conduct comprehensive assessments including psychosocial, clinical, and financial factors.
  • Facilitate planning and transitions of care, including referrals to outside specialty services and specialists.
  • Monitor patient progress and modify care plans as needed in collaboration with the healthcare team.
  • Ensure documentation is accurate, timely, and compliant with organizational policies and regulatory standards.
  • Advocate for patients to ensure quality care and resource access.
  • Educate patients regarding health conditions, care options, and community resources.
  • Support quality improvement initiatives and health programs as instituted by VBA Clinic healthcare team.
  • Serve in a nursing role in clinic as needed.
  • Perform other duties as assigned by Clinic Services Director.
Required Qualifications:
  • Strong commitment to VBA’s mission and philosophy of care.
  • Active State of Florida License as a Registered Nurse: Associate of Science in Nursing (ASN) required; BSN preferred.
  • Ability to function independently while leading and mentoring within the healthcare team.
  • Excellent oral and written communication skills.
  • Proficiency with electronic medical records, office equipment, and Microsoft Office Suite; experience with Amazing Charts is preferred.
  • Strong clinical judgement and ability to respond calmly and effectively in emergencies.
  • Ability to establish and maintain effective relationships with patients, staff, volunteers, and community partners.
Physical Requirements:
  • Ability to stand for extended periods, push, pull, reach, bend, stoop, and stretch.
  • Ability to operate a computer, photocopier, telephone, and other standard office equipment.
  • Occasionally lift and carry up to 20 pounds.
